Treatment
Foster Care (TFC) - Program serving youths ages 3-18 who are
suffering from severe emotional or behavioral disorders with 24-hour
supervision. This program is an alternative to residential treatment
facilities and in-patient hospitalization.

Behavioral Respite - A
service offered to families to allow a caregiver time off. This
service is offered to families on an as needed or emergency basis.
This service may be done in the caregiver's home or in the respite
providers home on a daily or hourly basis.

Individual, Group,
and Family Outpatient Therapy - Licensed therapists utilize client centered solutions to improve behavior and habits.

Case Management Services - A
service providing assistance with facilitating access to necessary
and quality care while advocating and building on the strengths and
needs of the client and their family.

Disability Medicaid Waiver (DD Waiver)

Family Living -
Also known as home-based service. We offer in-home placement of
developmentally disabled individuals who are carefully matched with
a provider that meets each individual's specific needs.

Substitute Care - Also known as Respite is an hourly service
offered to families to allow a caregiver time off.

Supported Living - 24-hour care in a group home
setting.

Independent Living
- Also known as Assisted Living, Services provided in a clients home
designed to increase or maintain independence and promote
self-advocacy.

Personal
Care - an hourly program that assist's the Client with
activities of daily living

Day Habilitation - Community based center with
activities and involvement designed for goal specific skill
building.

Supported Employment - Service designed for goal
specific skill building through community based activities and
involvement.

Community Access - This service provides
various supports to assist individuals in developing valued roles in
the community.
