Treatment Foster Care Coordinator
Coordinates the implementation of client treatment
plans.
Essential Functions and Responsibilities
-
Ensures that intake paperwork in complete
-
Development and implementation of the treatment plan
and discharge plan in accordance to certification standards.

Monitors client and his/her situation
-
Attends necessary appointments for clients
-
Assures that all treatment team members participate
in the treatment planning process
-
Supports client relationship with
biological,adoptive and foster families.
-
Prepares written monthly and quarterly contact
notes, reviews and assessments
Required Qualifications
-
Bachelor or Master's degree from an accredited
college or university in social work, psychology, family studies,
guidance and counseling or counseling.

3 professional/character references
-
Criminal records check
-
Master degree applicant must have at least 1 year
experience in the field

Bachelor degree applicant must have at least 2 years
experience in the field
-
Ability to communicate service needs clearly and
accurately in written and verbal formats.

Ability to supervise foster parents.
-
Excellent organizational skills.
Working Conditions and Physical Effort
-
Work is performed in a typical office environment,
as well as residences and the community.
-
Moderate physical activity. Requires handling of
average weight objects up to 75 pounds or standing, sitting and/or
walking for more that 4 hours per day.
