Treatment Foster Care Coordinator

Coordinates the implementation of client treatment plans.

Essential Functions and Responsibilities

  • Ensures that intake paperwork in complete

  • Development and implementation of the treatment plan and discharge plan in accordance to certification standards.

  • Monitors client and his/her situation

  • Attends necessary appointments for clients

  • Assures that all treatment team members participate in the treatment planning process

  • Supports client relationship with biological,adoptive and foster families.

  • Prepares written monthly and quarterly contact notes, reviews and assessments

Required Qualifications

  • Bachelor or Master's degree from an accredited college or university in social work, psychology, family studies, guidance and counseling or counseling.

  • 3 professional/character references

  • Criminal records check

  • Master degree applicant must have at least 1 year experience in the field

  • Bachelor degree applicant must have at least 2 years experience in the field

  • Ability to communicate service needs clearly and accurately in written and verbal formats.

  • Ability to supervise foster parents.

  • Excellent organizational skills.

Working Conditions and Physical Effort

  • Work is performed in a typical office environment, as well as residences and the community.

  • Moderate physical activity. Requires handling of average weight objects up to 75 pounds or standing, sitting and/or walking for more that 4 hours per day.
