Community Support Manager

Facilitates a client access to necessary care and the quality of such care using a medical and psycho-social service model that advocates for, and builds upon, a client's and family's strengths and needs; facilitate the use of a client's natural helping resources in the community; ensure continuity of care; promote coordination of physical, behavioral-health and social services; and problem-solve with clients, family care givers, legal guardians and other significant parties to assist them, if possible, in setting goals for themselves.

 
   
     
   

Essential Functions and Responsibilities

  • Meet State certification requirements for case management services.

  • Correctly complete all required reporting

  • Prepare case management assessments

  • Prepare service plains

  • Access, coordinate and link and monitor services with client

  • measure and evaluate outcomes and progress

  • Provide timely services in accordance with client and family needs

  • Maintain a case load of not more than 20 clients

  • Participate in skill enhancement as needed for professional discipline

Required Qualifications

  • Bachelor of higher degree in social work, counseling, psychology, sociology, education, special education, cultural anthropology, or a related health, social or human service field from an accredited college or university and 2 years of direct social service, human service or health experience serving the target population. OR

  • Current New Mexico licensure as a registered nurse (RN), or licensed as a practical nurse (LPN) as defined by the NM board of nursing, with a minimum of one year direct social service, human service, or health experience serving the target population. OR

  • An Associate degree and a minimum of 3 years experience in community health, social service or human service including 1 year experience serving the target population.  OR

  • A high school diploma (or GED) and a minimum of 4 years experience in community health, social services or human services including 1 year of that experience serving the target population; an individual with this qualification level must work under the direct supervision of an experienced case manager with the agency.

Working Conditions and Physical Effort

  • Work is performed in a typical office environment, as well as residences and the community.

  • Moderate physical activity. Requires handling of average weight objects up to 75 pounds or standing, sitting and/or walking for more that 4 hours per day.
